Root Cause Analysis

Root cause analysis helps to identify the ‘what’, ‘how’ and ‘why’ a failure has occurred, thus preventing recurrence.

Root causes are those, over which the ‘owner’ of the process has control. Root causes are specific, reasonably discernible situations that can be identified and mitigated by improved processes and / or recommend equipment.

The process of root cause analysis involves the collection of data, cause charting, root cause identification and effective recommendations to prevent future system failures.
